The state of Illinois has changed the requirement regarding 5th grade physicals. Physical exams are no longer required for students entering 5th grade BUT are now required for students entering 6th grade.
Dental
The state is now requiring that children in the current grades of K, 2 & 6 have a current dental form in their school medical file. ALL forms must be turned in to the school office by MAY 1, 2008. NO EXCEPTIONS!
